By booking this appointment and on the day of your appointment you verify that you have no fever or chills with fatigue, cough, difficulty breathing or shortness of breath, body aches, new loss of taste or smell, sore throat, congestion or runny nose, nausea or vomiting, diarrhea or other signs of acute illness. If you are experiencing theses symptoms please do not arrive for you appointment and rather, seek care from your primary physician to assess for COVID-19.
